The CSC team offers community-based outpatient treatment services for individuals ages 16-26 who are experiencing psychosis. Treatment is provided to individuals and their families in both individual and group settings. Services will be delivered primarily in-home/community and the team additionally offers an office-based clinic location. CSC is comprised of a multi-disciplinary team who work together to serve the individual and family through the NAVIGATE model, which emphasizes a shared decision-making approach with a focus on strengths and resiliency. 

 
 

Eligibility

Age: 16-26 years old

Diagnosis: The CSC team works with people who are exhibiting signs or symptoms of psychosis that are impacting their functioning.  May have diagnosis of Schizophrenia, Schizoaffective Disorder or Schizophreniform but formal diagnosis is not needed for referral.

Primary Care: Individuals are not required to have a Primary Care Physical (PCP) for referral to this service.

Location: This service is open to individuals in the Greater Fall River and New Bedford areas, including the following towns:

  • Acushnet

  • Dartmouth

  • Fairhaven

  • Fall River

  • Freetown

  • Marion

  • Mattapoisett

  • New Bedford

  • Onset

  • Rochester

  • Somerset

  • Swansea

  • Wareham

  • Westport

Prior authorization for DMH services or DMH application is not required. Spanish language capacity on team; all other languages provided by in-person and/or telephonic interpretation services.

Team-based, multi-component service including:

 
  • Psychiatric assessment & Psychopharmacology

  • Individual therapy & Individual Resiliency Training (IRT)

  • Family Support and Education

  • Peer Support Services

  • Care Coordination

  • Group therapy

  • Cognitive Behavior Therapy (CBT)

  • Supported employment and education services

  • Multi-Family Group Therapy

 

Cost

Service will be provided regardless of insurance coverage.  Team will work with individuals and families on payment options, as needed.
